Alert: Please submit all clinical requests via PATCHS from 20th January

We politely ask all of our patients with internet access to complete a PATCHS form (link on our homepage) if they require an appointment with one of our GPs or Advanced Nurse Practitioners. We have made this change in order to adjust to the new demands and pressures in General Practice, with the aim of dealing with medical requests safely and efficiently. If you are unable to access PATCHS online you can complete an appointment request form at the front desk or over the phone on 0151 638 8833. All PATCHS forms will be reviewed by our Care Navigation Team, with the support of a Duty GP, and clinically triaged and responded to within 48 hours.
