Register with our practice

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. If you have a form of photo ID and proof of address then please bring this with you, but this is not required in order to register with us and be eligible to receive healthcare.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. You can either come into the Practice to collect these paper forms or complete online via the link above.

You will need to seek on-going medical treatment from your current GP Practice until your new registration has been processed and accepted – this can take up to 7 working days.

Catchment Area (Practice Boundary)

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

Named Accountable GP

A Named Accountable GP is allocated to every registered patient at the practice. The GP is responsible for the co-ordination of all medical services and ensures they are diverted to patients where required. This is also for administrative purposes

You do not need to see the Named GP when booking an appointment patients are entitled to choose to see any GP,  Advanced Nurse Practitioner, Practice Nurse or any other Health Care Professional in the practice
