If you have a query regarding an insurance form or any other form the doctor has for signing, please contact the surgery between 10:00 – 15:00 Monday to Friday. This also includes collection of forms at the surgery. If you are unable to call in at this time alternative arrangements can be made.

NOTE: The preferred payment for this service is by cheque or cash – unfortunately we cannot accept payment by credit/debit card.
For any non-medical requests, patients are required to fill out a DSAR (Data Subject Access Request) form. Please either print from the link below, or come to our reception desk to collect a copy.
